Price Transparency

Community First Medical Center is providing a listing of its standard charges in accordance with
the Centers for Medicare and Medicaid Services.

The following list includes the charge description of the item or service and the charge amount as reflected in our hospital’s charge description master (CDM).

PLEASE NOTE THAT OUR CHARGES DO NOT INCLUDE FEES FROM YOUR SURGEON, ANESTHESIOLOGIST OR OTHER PROFESSIONAL SERVICES BILLED BY THE PHYSICIAN OR OTHER PROFESSIONAL PROVIDERS.  Typically, you will be billed separately for these professional services.

The starting list of charges is the same for every patient. The charges may vary by patient even though a similar procedure was performed. This may be due to the patient’s medical condition, length of time spent in surgery or recovery, complications requiring unanticipated procedures, kinds of medication needed, and whether the services were provided in an inpatient or outpatient setting.

These prices do not necessarily reflect what you will pay out-of-pocket. If you are insured, you will likely be responsible for a percentage of this cost. There are different factors that determine your cost for healthcare services you receive.

Patients with health coverage

  • Insurance coverage and benefits: This is the amount that your insurance provider has contractually agreed to pay CFMC for services you receive. Your benefits also explain the types of services that your insurer will cover.
  • Out-of-pocket costs: This is the portion of the cost of care that you are responsible for through co-pays, deductibles and co-insurance. Insurance providers typically define the maximum amount that a patient will pay in a given benefit year.  Contact your health insurance company to get an explanation of how much each service will cost, considering deductibles, co-pays and other related fees. Call the Financial Assistance Department at 773-794-7626, Monday through Friday, 8 a.m. – 4 p.m.The following publication, developed by the Healthcare Financial Management Association (HFMA), may also help you better understand the standard charges:
  • Patients without health coverage are protected by the state’s Hospital Uninsured Patient Discount Act. Under this legislation, eligible patients may qualify for free or discounted care. Our Financial Assistance Department is available to help patients with applying for assistance.
